September Monthly Review: Health MCP Guy?

For all the time I spend talking about APIs and data exchange, this newsletter has had embarrassingly little interoperability of its own. It’s old-school SaaS, baby - UI or bust. Not all of that is my fault! Substack, for all its popularity, is (as mentioned before) really frustrating as my system of record. The primary user it optimizes towards is the reader, whose attention feeds the app, the Notes feed, and the recommendation network that Substack is actually selling. Health API Guy is a reader-supported publication. To receive new posts and support my work, consider becoming a free or paid subscriber. Subscribe They’re falling into the same trap that Medium once sprinted into, albeit at a much more glacial speed. Younger generations may not remember (or never knew) it, but Medium once lured publications like The Ringer and Signal v. Noise onto its platform, only to lose them as it rebuilt itself around a paywall and an algorithmic feed. Substack’s original pitch circa 2020 was the antidote (you own your list), but every new feed feature chips away at it. I get it: writers are generic supply in the eyes of the Substack product team. Generic supply follows demand, so the consistent pressure on the PMs is to generate demand, which leads to every product cycle going toward discovery (Notes, recommendations, the app, live video). The end result lackluster author tools (laggy and broken analytics are truly crippling) all despite the premium price point. The archive is where it crosses from authorial gripes to actual reader impact, though. Across Substack and LinkedIn, I’ve published 1,218 posts and nearly 900,000 words since 2018. Substack’s native search can see only the 464 posts that live on Substack, and even there it’s woefully inadequate at surfacing relevant content (even for the guy who wrote it)! Readers have the same problem, writ large. Ask the Archive Fixing search (for both my readers and my own use) turned out to be extremely non-trivial, unfortunately: There’s no public API for readers to access content RSS exists for new content notification, but it only carries the 20 most recent posts and cuts paid ones off at the preview The only Substack MCP is an admin tool for analytics The export exists for authors, but it’s painfully awkward and slow. So I built something to work around that: the Health API Guy MCP. It’s a private MCP server holding the full text of everything I’ve written across Substack and LinkedIn. Add it to Claude or ChatGPT as a connector and you can ask things like: It pairs keyword search (for acronyms and case names, of which we have many) with semantic search. Every post is also labeled by topic, form, and court case, so you can pull a lawsuit’s (or multiple cases’) entire docket across Substack and LinkedIn in one query: Hail, Healthapiguyhydra This is also a bit of me eating my own cooking. I’ve spent the year arguing that headless healthcare is arriving and that developers want primitives over abstractions. Last week in Hail, Benihydra, I watched Salesforce plug its platform into Claude and bet that the records underneath outlive whatever interface sits on top: Salesforce must believe that the Hydra’s immortal head is the records, permissions, and business logic (in that they will survive whatever happens to the interface). It felt a bit hypocritical to keep my own archive behind a Substack search bar. The archive is the body. Substack is one head, and now whatever tool you might use is another. Will the EHRs follow my lead? Probably not imminently, but one can hope! Why Founding Members Only The MCP is available to Founding Member subscribers. A few reasons: It costs money and time to run. Hosting, the database, Clerk usage and AI compute for search and labeling are significant operational costs (that increase with usage). Substack gives me no way to automate access. There’s no API to check whether someone is a paid subscriber, so every user is manual operations for now. It’s a beta. We’ll need to improve things and parts will break. I’d rather they break for a small group of dedicated readers who will tell me about it. The Founding tier deserved a real benefit. Until now it was mostly a generous tip. Now it comes with something. Month in Review Articles Published Make Networks Dumb Again (Sep 11): AI is making old analog networks cheaper to use and flooding them with machine traffic in the process. A plea for agent-native rails with the intelligence at the endpoints, and for Washington to drag them to ubiquity. Interoperability, Unbuttoned (Sep 29): Interoperability is a series of workflows in a trenchcoat, and this post finally takes the trenchcoat off. A color-coded map shows which provider workflows went digital, which are fragmented, and which still run on fax. Video Content Antitrust’s System of Record Problem (Sep 3): The FTC’s investigation into Epic prompted a ninety-minute webinar, now posted with chapters and a smoothed transcript. We get into why market definition, missing B2B share data, and the SSNIP test leave antitrust poorly suited to systems of record, plus why information blocking may be doing more than any pending lawsuit. The Information Exchange: Cerner Milking Edition (Sep 18): This was so fucking fun. Ryan Tucker, Ryan Brickner, Brad Thorson, and I try an Around the Horn format, complete with points, penalties, and a mute button. We debate whether Oracle Health is dead, who would actually buy Cerner (IBM enters the chat), p(Doom), and why the front desk still hands you a clipboard. Regulatory Ask Not What Your Network Can Do (Sep 23): CDC’s RFI on public health data intermediaries lists use cases already served, unevenly, by eCR, lab reporting, IZ Gateway, and syndromic surveillance. The real opportunity sits in one question about AI: a network that can handle the next emergency without another interface project. Court cases Particle v. Epic: The Naughty Schoolteacher (Sep 4): After a summer-long vigil, the answer on Epic’s early summary judgment bid is two paragraphs and a remedial assignment. Particle advances to the next grade, and market definition gets another year of discovery. The Epic Discovery Files (Sep 8): Texas’s motions to compel arrive with a 251-page appendix of requests, objections, and meet-and-confer letters with Cravath. Inside: Epic using generative AI for document review, denied interoperability requests in scope, and Noerr-Pennington raised against lobbying discovery. Epic v. Health Gorilla: “Working as Designed” (Sep 9): Epic wants nothing to do with the data breach MDL, and explaining why puts fresh discovery on the public docket. The spiciest allegation: Health Gorilla told GuardDog to scrub its law firm business from its website instead of cutting off access. Veeva v. Epic: Legal Bypass Surgery (Sep 15): Veeva asks the Wisconsin Supreme Court to skip the Court of Appeals, while law professors and three policy groups line up as amici. Meanwhile, Epic’s lawyers apologize for another round of citation errors, including a quote that never existed. Texas v. Epic: Behind the Black Bars (Sep 17): The court portal served up Epic’s unredacted filings, revealing a $5 million document review budget and the AI workflow behind it. Also inside: the 200-plus company names Texas wants searched, and one very expensive Zoom invitation. OpenEvidence v. Doximity: Consequences Will Follow (Sep 25): OpenEvidence tells the court it will not comply with a discovery order, and its own lawyers at Quinn Emanuel say they won’t defend the choice. Add years of Slack reported unrecoverable, and Rule 37 sanctions come into view. Health Tech Keeps Choosing Violence (Sep 28): Doximity moves to end OpenEvidence’s case, Epic asks Texas to show its work, and a sleepy Audacious Inquiry patent fight sprouts antitrust and information blocking counterclaims. The main event is a long-sealed Cognizant v. Infosys ruling that hints where system of record antitrust claims can survive. EHRs One Verb for Oracle Health (Sep 14): Oracle’s earnings call quantifies every data center to the decimal point, while Oracle Health gets a single verb: “accelerate.” Panning the few healthcare mentions turns up an agentic care management system, a research-to-care ledger play, and the question of whether healthcare gets Larry’s patience. Epic’s Forretress Under Siege (Sep 30): Judy Faulkner tells a panel Epic paused hundreds of projects to harden its software and calls it a “shame,” while a spokesperson insists the roadmap hasn’t changed. Project Glasswing, a Pokémon-branded QAN sprint, and a trove of Jodel grievances reconcile the two Epics and preview the Jevons paradox coming for all software. Industry Analysis Scanning for a Network (Sep 1): Scan.com raises $220M to build the imaging network labs never got, speedrunning Zocdoc’s consumer-to-B2B pivot. Its wedge to network density may be the most American one available: personal injury litigation. The Front Desk Wants the Damn Whole Building (Sep 16): Hello Patient buys Converse Health, pushing its AI front desk into referrals, chart work, and prior auth. Ukraine’s Academy of Sciences warns against Russian cooperation after attack

The National Academy of Sciences of Ukraine has urged the international scientific community not to “restore or legitimise institutional cooperation with Russian state scientific and educational structures,” after a Russian drone hit its Presidium building in central Kyiv, killing at least two people and injuring several others. The drone strike took place in the early afternoon on September 28 while staff were working.The academy called the strike, one of several Russian attacks on Ukraine’s capital using jet-powered drones in recent days, an act of terrorism. “It is a strike [. . .] on the very idea that knowledge, research, and the search for truth should serve life, not its destruction,” it said in a statement.The academy thanked the global scientific community for its solidarity following the attack and warned against any temptation to normalise scientific relations with Russia.“Science cannot exist outside of moral responsibility. There can be no ‘ordinary scientific cooperation’ with a state that kills people and destroys scientific institutions of another country,” it said.“We urge not to create opportunities for Russian state institutions to return to the international scientific community as if nothing happened. Please Stop Conflating Sports Betting Handle With Spending On Entertainment

Which means it’s time to get ahead of a ridiculous narrative before it picks up steam on social media or in mainstream media. The headline and start of the story tell the same narrative (emphasis added): Gambling becomes America’s favorite pastime as Americans spend more on sports bets than movies, arts, museums, and music combined: The North American box office totaled $8.87 billion in 2025, which is still 22% below pre-pandemic levels. Recorded music revenue hit a record $11.5 billion. Live music like concerts and festivals brought in $18.51 billion. Meanwhile, book publishers tracked by the Association of American Publishers reported $14.6 billion for the year. And the U.S. museum industry generated an estimated $16.4 billion. Add it up and the total comes to roughly $70 billion, which is less than half what Americans wagered on sports. The story, to its credit, goes on to tell us why sports betting is entirely different, although people who only read headlines will miss that nuance. (The tweet above definitely skips over the nuance.) I will mostly be preaching to the choir here, but it’s worth saying out loud: Wagering money on sports is not the same thing as spending on entertainment from a financial perspective. When you spend money on a movie or concert ticket, that money is gone. You exchanged money for an experience. It’s a good trade! I get to have fun for “x” dollars. (Unless the movie sucks, I guess.) When you bet money on sports, the money is not necessarily gone every time you wager it. It could be gone, but sometimes it’s not and you actually get more money back! That’s not “spending” money or something you should try to compare directly to entertainment spend. A far better comp would be how much people lose at sports betting. That number was more like $17 billion in the US last year, at least for state-regulated sportsbooks. Why are we trying to equate handle and entertainment spending? I dunno. It makes for splashy headlines, I guess. Now, people should be betting for entertainment, and not because they expect to win in the long term; no doubt about it. Anyway, here’s hoping I don’t see people saying Americans “spent” hundreds of millions of dollars on sports betting this fall. The US ‘golden age’ science strategy: what Europeans need to know

The US has unveiled what officials claim is the most radical overhaul of the public R&D system in a generation, which if implemented could have significant implications for transatlantic research collaboration. Science: A New Golden Age derides universities as “legacy institutions” and instead wants to prioritise funding on technological challenges, new research labs and funding that follows individual scientists, rather than being linked to their institutions. “Too much of our research enterprise has come to serve itself rather than the scientists within it,” writes Michael Kratsios, director of the White House’s Office of Science and Technology Policy (OSTP), the only named author of the report. “Federal funding agencies should support a broader range of performers.” But some critics argue that the strategy is a smokescreen, using genuine problems in academia as an excuse to further weaken US universities, which US vice-president JD Vance has labelled “the enemy..”Even as it talks of a “golden age” for science, the White House has proposed halving the 2027 budget of the US National Science Foundation (NSF), and has put forward double-digit cuts for other funding bodies, such as the National Institutes for Health. It asked for similarly steep savings in 2026, but Congress stopped them going through. Science: A New Golden Age says success will be measured by the extent to which scientists are “free” to pursue the most “vital questions,,” painting academia as currently hidebound by bureaucracy, sluggishness and consensual conservatism. And yet the Trump administration is simultaneously planning new rules that will hand political appointees unprecedented powers to screen and cancel grants. The report has pretentions to be the successor to Science: The Endless Frontier, the seminal 1945 document in which presidential science advisor Vannevar Bush set out the case for post-war government funding of basic research, helping to spur the creation of funders such as the NSF. Even the title page font is the same. But it remains unclear if the latest report will leave anything like as lasting a legacy.Here, Science|Business explains some of the biggest proposed changes in Science: A New Golden Age, and how Europeans think it might change the landscape of their biggest science partner.Universities under fireA key target of the report are US universities, which it repeatedly suggests are outmoded as research institutions. It criticises the NSF for channelling resources “overwhelmingly to a single type of performer, the university-based, principal investigator-led research group..” Universities “extract” money from researchers’ grants to support “growing administrative bloat,” alongside legitimate uses. The Trump administration has been trying to cut the overhead costs in federal research grants that flow to universities. Instead, new institutions are being built. In May, the NSF announced it would spend $1.5 billion on two new so-called X-Labs, focused on scientific instrumentation and quantum systems. The idea is that they fill a niche in the landscape, tackling problems that are too big for a university lab, but too early stage for private start-up funding. Parallel with Europe?This shift to new research organisations parallels moves in Europe to set up new innovation agencies, such as Germany’s Sprind, the UK’s Advanced Research and Invention Agency, and the EU’s European Innovation Council. The Dutch and French governments are looking to set up their own entities too. There’s now an emerging consensus, evident in a number of new strategies, that public R&D needs to be shaken up, Charles McIvor, an economist at the OECD, has written in response to Kratsios’s plans. “What stands out for me across these strategies [in the US, Germany, South Korea and Japan] is that governments are realising that the old ways of delivering STI [science, technology and innovation] programming no longer works,” he said in a LinkedIn post. Metascience wins?The document is, at least on the face of it, also a big win for some policy thinkers in the US who want far more experimentation with new forms of research grant and institution.It name-checks a litany of new public R&D ideas, from focused research organisations and advance market commitments to “golden tickets,” which allow grant reviewers the power to hand applicants money without requiring the consensus of other review panellists.Related articlesEuropean researchers sound the alarm that Trump rule changes will hit cooperationFocused research organisations: what are they, and how do you set them up? NIH turmoil risks reciprocal EU-US access to health research funding In addition, every federal research funding agency should set up a metascience unit, the strategy says, “with authority to run controlled experiments on review and funding mechanisms.” The UK has already set one up, the strategy points out. This mirrors recommendations to the European Commission. In 2024, a report led by former Portuguese science minister Manuel Heitor also advised the EU to set up an “experimental unit” to test out new methods of funding research. So far, however, Brussels hasn’t put forward any plans to do so. Bypass universitiesBut there’s a worry that the Trump administration is using the metascience movement to justify an overhaul of public R&D that punishes universities, which it sees as centres of political resistance. Last year, it threatened to cut Harvard University’s federal funding unless it hired and admitted a “critical mass” of new faculty and students to create “viewpoint diversity” across campus. It’s also attempted to strip academics of funding for working on politically forbidden areas, like LGBTIQ people’s health.The new strategy appears to be a “move to bypass the power of universities full stop and allow funders and political bureaucracies to handpick the projects and people who will execute them,” said Jean-Claude Burgelman, a former senior official in the Commission’s research directorate, on LinkedIn.The EU’s science counsellor in Washington, Florent Bernard, wrote that the plans risk diverting public research funding so that it “solely serves private interests.” “Who, if not independent universities, will do research that demonstrates the impact of tobacco on lung cancer or the role of human activities in our current climate crisis?” he wrote.Should we take it seriously?“I would view the overall document as a bad faith argument,” said Cole Donovan, director of science policy at the campaigning group Stand Up for Science, and a former OSTP official. “I don’t see the project itself as something that can be isolated from the broader attack on the system.” He thinks the argument that academia has become dysfunctional and in need of radical reform is being deployed as part of a “PR argument” to legitimate moves against US scientists. The strategy presents ideas like metascience units as new, whereas there were moves to set them up at the NSF during the previous administration of Joe Biden, Donovan said. The other caveat here is that the OSTP has limited power to carry out any of these recommendations. While Kratsios has issued a memo to research agencies telling them to implement the recommendations in the report, he doesn’t have direct budgetary control. “As a former OSTPer myself, this does not seem like a document that is realistic,” said Donovan.
