United States and Madagascar Discuss Five-Year Health MOU Implementation Plan  - U.S. Embassy in Madagascar

United States and Madagascar Discuss Five-Year Health MOU Implementation Plan The United States and Madagascar’s Ministry of Public Health are in the process of developing a plan for implementing the U.S.-Madagascar five-year Memorandum of Understanding (MOU) on health cooperation, signed under the Trump Administration’s America First Global Health Strategy. The two-day workshop, at the U.S. Embassy in Antananarivo, brought together technical experts and senior leadership from the Malagasy and U.S. governments to discuss a shared vision through a preliminary draft MOU Implementation Plan and to establish the joint governance structures that will guide U.S. foreign assistance in health over the next five years. The jointly decided bilateral health MOU, signed on December 22, 2025, builds on decades of U.S. global health assistance to Madagascar. The health MOU involves the U.S.’s plans, working with Congress, to provide approximately $134 million (over 554 billion Ariary) and a Malagasy commitment of $41 million (over 169 billion Ariary). It aims to strengthen Madagascar’s health system, improve disease prevention and response, and support services that address health security concerns for both countries, including malaria, maternal and child health, polio, and global health security issues that could reach U.S. shores if not contained at their source. During the recent workshop, technical staff from the Ministry of Public Health and the U.S. Government health team worked in thematic groups to draft detailed implementation plans for the five-year MOU period. On the second day, U.S. Chargé d’Affaires Stephanie Arnold and Ministry of Public Health and Economic and Finance Ministry leadership joined the technical teams to discuss plans and proposed terms of reference for two committees: a Joint Steering Committee and an Operational Committee. These structures will ensure clear roles and responsibilities, regular dialogue between the two governments, consistent monitoring of progress, and accountability for results. The framework includes performance benchmarks that will guide continued U.S. support and track Madagascar’s progress toward self-reliance. “We see this not as a U.S. project, but as a joint framework under Malagasy leadership, with U.S. support,” said U.S. Chargé d’Affaires Stephanie Arnold during the workshop’s opening session. The approach reflects U.S. foreign assistance principles that emphasize strategic, results-driven support that advances American interests while helping recipient countries build sustainable health systems. The framework is designed to build systems that Madagascar will increasingly finance and lead itself, reducing long-term dependence on foreign assistance.

Jiang Feng: The Unravelling of German & European Politics

A state election in eastern Germany has become a revealing test of the changing political landscapes of Germany and Europe, writes Jiang Feng, Professor and Chairman of the Council of the Shanghai Academy of Global Governance and Area Studies (SAGGAS) at Shanghai International Studies University. His analysis follows the decisive victory of the Alternative for Germany (AfD) (which has been classified as right-wing extremist by German authorities)in the state of Saxony-Anhalt on 6 September, in which it finished far ahead of its mainstream rivals. Jiang previously served as Minister-Counsellor at the Chinese Embassy in Berlin. The following article was published on 8 September 2026 on huanqiu.com, the online platform of the Global Times. On 6 September, the results of the state election in Saxony-Anhalt, Germany, were announced. The Alternative for Germany (AfD), classified as far-right by German authorities, made history by becoming the largest party in the state parliament, winning 43.8 per cent of the vote — twice its share in the previous election. The Christian Democratic Union (CDU), which had governed the state continuously for 24 years, won just 17.2 per cent, its worst result ever in Saxony-Anhalt. On the night the results were announced, the atmosphere at the CDU gathering was unusually calm. There was neither shock nor argument, as if the defeat had long been expected. Several veteran party members who had spent years working in the state government said: “Defeat is not a bad thing. It is time to make changes.” This state election has not only upended Saxony-Anhalt’s political landscape, but also laid bare the deep tensions between Germany’s traditional party structure and model of governance, on the one hand, and an increasingly polarised society on the other. Its repercussions are already being felt at the federal level, making Saxony-Anhalt a useful case study of the broader trend towards political polarisation and realignment in Europe. Governing Difficulties after Electoral Victory The most immediate consequence of the election is a historic setback for the mainstream party order that took shape in post-war Germany. In Saxony-Anhalt, the AfD has leapt from a peripheral party to the largest political force in the state. Its electoral base has expanded beyond the traditional category of “marginal protest voters” to take on the characteristics of a cross-class coalition. The party has not only consolidated its support in rural areas, but also attracted many middle-class voters and even members of the social elite with stable jobs, their own homes, and substantial incomes. It has thus evolved from a “protest party” into one with the potential to govern. Despite its commanding lead in the vote, however, the AfD faces an institutional obstacle to governing the state: the mainstream parties’ long-standing “firewall” against it. Neither of the two potential routes to forming a government appears likely to break the current political deadlock. The first would be a coalition of establishment parties, led by the CDU and bringing together the Social Democratic Party (SPD), the Greens, the Left Party (Die Linke) and the Sahra Wagenknecht Alliance (BSW) in a cross-camp government. In terms of parliamentary arithmetic, such a coalition could prevent the AfD from forming a government and preserve the outward stability of the traditional political order. Yet these parties are fundamentally divided over key issues including immigration, the energy transition, economic management, and cultural and education policy. Coalition negotiations would therefore be extremely difficult, and even if a government could be formed, it would most likely prove short-lived. More importantly, an alliance of establishment parties aimed at keeping the largest party out of government would play directly into the AfD’s narrative that “political elites are suppressing public opinion”. It could deepen perceptions of political unfairness, further strengthening the AfD’s political capital, and creating a dynamic in which “the more it is suppressed, the more people vote for it”. Ulrich Siegmund, the AfD’s candidate for state premier, has said that if efforts to form a government fail because other parties refuse to cooperate, the AfD will push for a fresh election, which he believes could deliver the party an absolute majority of seats. This points to more intense political battles ahead. The second option would be a coalition between the AfD and the BSW. Together, the two parties would command a simple majority in parliament, making this, on paper, the quickest route to forming a government. The BSW is the only elected party that has not ruled out cooperation with the AfD, and the two parties share some common ground, including on policy towards Russia. There is therefore a degree of political basis for cooperation. Were such a coalition to materialise, Germany would see its first state government jointly formed by a party defined as far-right and one with a far-left background, creating what might be called a “bipolar anti-establishment alliance”. Such an outcome would fundamentally break with the ideological boundaries of post-war German party politics. It is worth noting that even if the AfD ultimately fails to enter the state government, its roughly 47 per cent share of parliamentary seats would give it a powerful blocking minority. Without its consent, major decisions requiring a two-thirds majority — including constitutional amendments, significant budgetary adjustments, and state-level institutional reforms — would be extremely difficult to pass. Therefore, whoever ultimately forms the government, the AfD will be an unavoidable force in Saxony-Anhalt politics, with a direct influence on the state’s legislative process. Systemic Failures in Germany’s Governance Model The Saxony-Anhalt election result reflects a systemic failure of Germany’s post-war mainstream-party model of governance to adapt to rapidly changing socioeconomic conditions. It stems from multiple, mutually reinforcing structural tensions and highlights the existing political system’s difficulty in adapting to “an increasingly polarised society First, collective anxiety has replaced individual material hardship as a central driver of political mobilisation. The rise of the AfD can no longer be explained simply by poverty or social marginalisation. Its increasingly cross-class electoral base suggests that what is driving voters towards the party is, above all, concern about the broader direction of society. This anxiety about the future of the political community goes beyond individual gains and losses. In political discourse, economic anxiety has become closely intertwined with immigration, while immigration and refugee issues are, in essence, questions of social fairness. This perception turns conflicts over the distribution of social resources into negative assessments of governing parties, providing a broad social basis for far-right political narratives. Second, regional disparities and East German identity have undermined the strategy of addressing eastern Germany’s structural challenges at the regional level. More than three decades after German reunification, eastern states such as Saxony-Anhalt have experienced periods of economic growth, but their underlying economic structures remain weak, and household incomes and wealth remain significantly below those in western Germany. At the same time, they continue to face population outflow, an ageing population, labour shortages, and other structural challenges. At a deeper level, historical experiences — including the collapse of the former East German economic system, mass unemployment during the transition, and the westward shift of decision-making power over major issues — have left some residents with a sense of being “second-class citizens” and of being excluded. For years, the German government treated the “syndrome” of reunification as a regional problem to be gradually addressed at the local level. That approach is becoming increasingly unsustainable, as regional development disparities emerge as a core structural factor shaping national politics. Third, the erosion of institutional trust is perhaps the most decisive factor. AfD supporters tend to distrust establishment institutions, including parliament, government, and the mainstream media. Their concerns about economic security, pensions, and social fairness are significantly greater than the social average. Increasingly, their support for the AfD reflects not an expectation that the existing system can be improved, but a search for an alternative to the current political order. Such a systemic deficit of trust will be extremely difficult for the traditional parties to repair in the short term. Fourth, a generational shift in political mobilisation has amplified political polarisation. Unlike traditional parties, which rely heavily on offline rallies and mainstream media coverage, the AfD has developed a social-media mobilisation strategy built around “raising an issue — identifying who is responsible — offering a simplified solution — building a shared identity”. It turns complex social tensions into concise political narratives, rapidly channelling public sentiment into political support. The contrast on the eve of the election was particularly telling. The establishment parties, together with trade unions and churches, organised an offline march attended by more than 10,000 people, but its reach was largely confined to those present and to traditional media coverage. The AfD held a much smaller indoor event, yet livestreaming on TikTok, YouTube, and other platforms, combined with the involvement of online influencers, gave it a reach and mobilising capacity far beyond that of the march. The contrast illustrates how the traditional parties’ methods of political mobilisation are increasingly struggling to keep pace with a political environment transformed by changes in media technology. A Microcosm of Structural Political Change in Europe The shockwaves from the Saxony-Anhalt election quickly travelled beyond the state to the federal level. Calls for political accountability followed immediately after the results were announced. The governing CDU and SPD are now under enormous pressure, while Chancellor Friedrich Merz and Vice Chancellor Lars Klingbeil face an unprecedented test. Political polarisation in Germany is no longer simply a regional phenomenon. The post-war model of multiparty government by establishment parties is breaking down and being reshaped. Viewed in a broader European context, the Saxony-Anhalt election is part of a continent-wide wave of party polarisation, mainstream-party decline, and rising right-wing forces. Far from an isolated phenomenon, it is a microcosm of the structural political changes unfolding across Europe. Weak economic growth, cultural tensions over immigration, and declining trust in institutions are fuelling the rise of right-wing forces in several countries, challenging the liberal internationalist order established in post-war Europe. As a core member state of the European Union, Germany’s ability to adapt its political system and the direction of its politics will have profound implications for both European integration and the evolution of the international order. Strengthening U.S.-Nigerian Health Cooperation under the America First Global Health Strategy - U.S. Embassy and Consulate in Nigeria

Strengthening U.S.-Nigerian Health Cooperation under the America First Global Health Strategy The United States signed a five-year, $5.1 billion bilateral health cooperation Memorandum of Understanding (MOU) with the Federal Republic of Nigeria December 19 to advance the America First Global Health Strategy, supporting resilient, self-reliant, and durable health systems while promoting accountability and shared responsibility. Under the five-year MOU, the United States intends to commit nearly $2.1 billion in health assistance, with nearly $3.0 billion in new domestic health expenditures by the Government of Nigeria over the same five-year period. This represents the largest co-investment any country has made to date under the America First Global Health Strategy and underscore Nigeria’s commitment to greater national ownership of its health system. Under the MOU, the United States will continue to support surveillance and outbreak response, laboratory systems, health commodities; frontline healthcare workers, and data systems. Nigeria faces significant health challenges, including one of the highest maternal and child mortality rates globally and approximately30 percent of the global malaria burden. U.S. assistance under the MOU will expand access to affordable, preventive and curative services for HIV/AIDS, tuberculosis (TB), malaria, polio, and maternal and child health, strengthening health outcomes across Nigeria. The MOU places a strong emphasis on Christian faith-based healthcare providers, recognizing their indispensable role in delivering care to communities in need. Nigeria’s more than 900 faith-based clinics and hospitals serve more than 30 percent of the country’s 230 million people, often in areas where healthcare facilities are limited or absent. The MOU provides approximately $200 million in dedicated support to strengthen and support these Christian facilities, enhance workforce capacity, and expand access to integrated HIV, TB, malaria, and maternal child health services. Investments in Christian faith-based health institutions are uniquely positioned to complement public-sector facilities and reinforce Nigeria’s overall health infrastructure. The MOU was negotiated in connection with reforms undertaken by the Government of Nigeria to prioritize the protection of Christian populations from extremist violence. As with all U.S. foreign assistance, the President and Secretary of State retain the right to pause or terminate programs that do not align with U.S. national interests, and the United States expects Nigeria to continue making measurable progress in combating religiously motivated violence against Christian communities. This five-year MOU is the latest of several health cooperation MOUs signed in Africa this month. The United States will continue to sign multi-year Bilateral Agreements on Global Health Cooperation with countries receiving U.S. health assistance to advance the America First Global Health Strategy.

Travel health insurance

Travel health insurance Do you have a friend or relative traveling to the Dominican Republic? Encourage them to get travel health insurance before they depart the U.S. to avoid possible medical emergencies becoming financial emergencies, too. Many travelers ask, “Why would I need travel health insurance–I have health insurance?” But your domestic U.S. health insurance may not work overseas. Hospitals here in the Dominican Republic often require payment in full prior to the patient being treated or being discharged. Some private U.S. insurance companies pay for overseas medical care, but usually only on a reimbursable basis, meaning you must pay first and request reimbursement later. The U.S. government does not pay for medical care overseas, and Medicare does not cover medical or hospital expenses incurred outside the United States. In addition, if the traveler needs to be medically evacuated for any reason, this is not covered by most U.S insurance and usually costs well over $10,000 USD. Travel medical insurance can alleviate this cost burden. Don’t let unexpected medical costs ruin your vacation! Please review more information here on Travel.State.Gov and on the Embassy website’s page on Medical Emergency – U.S. Embassy in the Dominican Republic.

Handover of Health Commodities

Joint Handover of Health Commodities Speech of Chargé d’Affaires Steve Bremner (…) Thank you for joining us. I am here today to demonstrate how U.S. foreign assistance under President Trump’s America First Global Health Strategy and our bilateral health MOU makes America safer and stronger while helping Madagascar build a more self-reliant health system. Through our bilateral Global Health Memorandum of Understanding (MOU) signed in December 2025 with the Ministry of Public Health, our two governments outlined shared priorities and responsibilities, including a strong focus on infectious disease threats and Madagascar’s increasing ownership and co-investment in its own health system. The MOU outlines the structure and terms of U.S. support to Madagascar’s health sector from 2026 through 2030, with a total commitment of $134,305,000 from the U.S. government and $41,216,252 from the Government of Madagascar to work jointly to address malaria, maternal and child health, polio and global health security issues. Stronger health systems help prevent instability and reduce the risk that infectious diseases reach the United States, making both our countries safer. America First foreign assistance is clear and disciplined: It is a tool of strategic engagement, not global charity. Every U.S. taxpayer dollar must show measurable results, reduce long-term dependence on U.S. taxpayer resources, and support greater self-reliance in recipient countries. Support must be efficient and targeted, with zero tolerance for fraud, waste, or abuse. This year, through the U.S.-funded Global Health Supply Chain Program – Procurement and Supply Management project, we are distributing: 989,250 long-lasting insecticide-treated nets, worth $2,000,000 to 63 districts and 1,661 Community Health Centers across Madagascar, with the third and final delivery of these nets having arrived in late June. Through the U.S.-funded Momentum Country and Global Leadership (MCGL) project, we are funding: The full cost of Starlink internet connectivity equipment procurement, installation, and one-year subscription for 31 priority District and Regional Health Offices. Through the U.S.-funded Strengthening Infectious Disease Detection Systems project, we are supplying: 908,523 pieces of critical medical equipment and a modernized digital surveillance system to the Ministry of Public Health. These materials will strengthen health systems, improve response capabilities, and help those most in need of protection from mosquito-borne diseases — particularly malaria, which continues to claim the lives of children and women at an unacceptable rate. Deployed effectively, they will contribute to reducing maternal and newborn mortality in rural and hard-to-reach communities where the need is greatest. In these same underserved areas, Starlink connectivity will play a critical role in enabling the rapid transmission of health data and information to central levels, ensuring that decision-makers receive timely, actionable intelligence and can mount swift, coordinated responses to protect populations. For Americans, this support delivers concrete benefits. Stronger health systems in Madagascar help detect and contain infectious diseases before they spread across borders, protecting American and Malagasy families. More stable, healthier countries can expand trade and economic cooperation with the United States. Our goal is not permanent support; it is temporary, targeted assistance that helps Madagascar save lives while achieving greater self-reliance and independence from foreign assistance. To the health workers and community health agents here that will use the products we’re delivering today: your dedication and skills save lives. Your work exemplifies the kind of local ownership that will enable Madagascar to reduce dependence on foreign assistance and manage the country’s health challenges. This targeted support delivers results today while helping Madagascar build toward a future where it can sustain and expand those health gains independently, ensuring that today’s U.S. support leads to Madagascar’s self-sufficiency. Thank you, Misaotra.

Cuba: US Embassy issues health alert due to GI pathogens like norovirus, Shigella

The US Embassy in Havana issued a health alert last week after seeing a significant increase in diarrheal illness across Cuba, associated with the continuing degradation of the water and energy infrastructure. This degradation affects water supply, and food storage and temperature control. Recent cases have included E. coli, norovirus, Shigella, and other gastrointestinal pathogens. This comes after a health alert dated August 28 which noted that Cuba’s water delivery infrastructure is increasingly unstable. The Embassy is aware of reports of residential neighborhoods that have gone for long periods without receiving water from municipal or public sources. Dengue cases surge in the Tampa Bay area Prolonged and worsening water issues and power outages caused by the regime’s mismanagement of public infrastructure impact the entire island. Water often only flows to residences when city power is on for a sufficient amount of time. Media reports indicate wells are running dry across the country due to drought, aggravating the ability to supplement water delivery. The lack of water, coupled with power shortages that have disrupted cold chain networks, can present significant health and safety challenges. We remind U.S. citizens to be aware of the risks when traveling to Cuba and take precautions. Embassy officials offer the following precautions to prevent illnesses: Avoiding Foodborne Illness: Be cautious of meat, poultry, seafood, eggs, dairy, and other perishable foods that may have been inadequately stored. Avoid foods that appear to have been sitting at room temperature for extended periods. Avoid raw or improperly washed vegetables. Be cautious of food cooked and then cooled, stored, or reheated, as improper temperature control can allow pathogens or toxins to develop. Wash hands thoroughly with soap and water before eating and after using the restroom. Hand sanitizer may be less effective against certain gastrointestinal pathogens, including norovirus. Use bottled or appropriately treated water; check that bottle seals are intact before opening. Avoid ice unless the water source is known to be safe. Do not use untreated tap water to prepare beverages or food. Be aware that contamination can occur when food, utensils, and surfaces are washed with contaminated water. If You Become Ill: Stay hydrated, especially with frequent diarrhea or vomiting. Most illnesses are self-limited and improve within several days. Seek medical attention for persistent or severe diarrhea, bloody stools, high fever, significant abdominal pain, repeated vomiting, signs of dehydration, or symptoms not improving. Common medications (e.g., acetaminophen, ibuprofen, antibiotics) are frequently unavailable in Cuba; consider bringing your own supply. Merck Life Science Expands Bengaluru Manufacturing with €13 Million Investment

Merck Life Science is expanding its manufacturing capabilities in Bengaluru with an investment of approximately €13 million, strengthening the company’s ability to supply critical materials to customers across India and the wider Asia-Pacific region. The expansion at Merck Life Science’s Jigani site in Bengaluru will introduce biological buffer manufacturing alongside the facility’s established chemistry capabilities. The investment is designed to enable faster and more reliable access to critical materials used by customers across research, diagnostics, biotechnology and biopharmaceutical manufacturing. The development further advances Merck Life Science’s “in-region, for-region” approach, under which the company is expanding manufacturing capabilities closer to the markets and customers it serves. By adding biological buffer production in India, the Jigani expansion will broaden the site’s manufacturing capabilities while strengthening Merck Life Science’s regional supply infrastructure for the rapidly developing life sciences sector across India and Asia-Pacific. The company said the expansion reflects both the contribution of its teams and its collaboration with government and industry stakeholders. Merck Life Science acknowledged the support of India’s Department for Promotion of Industry and Internal Trade (DPIIT), the German Embassy New Delhi, and Invest India in helping strengthen life science manufacturing capabilities in the region. The investment comes as India continues to expand its role across pharmaceutical, biotechnology, diagnostics and biomanufacturing value chains, increasing demand for dependable regional access to materials supporting research and manufacturing operations. The Bengaluru expansion reinforces Merck Life Science’s focus on building manufacturing capacity within Asia-Pacific and supporting customers through a more regionalised supply network.

Morocco health alert issued due to diphtheria: US Embassy

In the North African country of Morocco, the country’s Ministry of Health reported a small diphtheria outbreak with clusters in multiple regions in Morocco in late September. Moroccan health teams are tracing contacts and providing preventive treatment and vaccines. This has prompted the US Embassy to issue a health alert for U.S. citizens in Morocco. Diphtheria is a serious bacterial infection. It spreads primarily from person to person through coughs and sneezes (respiratory droplets). Most Americans were vaccinated as children and have received Td or TdaP boosters as adults. The risk to U.S. citizens in Morocco remains low. U.S. Citizens may contact their local doctors if they would like immunizations. Embassy officials advise citizens to watch out for the following symptoms: Sore throat or hoarse voice, mild fever, a thick gray or white coating in the throat or nose, swollen neck and trouble swallowing or breathing. If you or a family member has trouble breathing or swallowing, please contact your local doctor. They also advise on the following actions concerning vaccines: Check your vaccination records and your family members’ records. Get a booster if you need one. Adults need a diphtheria booster (Td or Tdap) every 10 years. Make sure your children are up to date on their routine vaccines. Outbreak News Today is a reader-supported publication. U.S. Delivers Maternal Health Equipment Across Madagascar - U.S. Embassy in Madagascar

U.S. Delivers Maternal Health Equipment Across Madagascar U.S. Chargé d’Affaires Stephanie Arnold today provided critical maternal and newborn health equipment that will reach healthcare facilities across Madagascar, demonstrating how U.S. foreign assistance builds a safer, healthier world by helping recipient countries build self-reliant health systems in line with the America First Global Health Strategy. Under President Trump’s America First foreign assistance policy and the America First Global Health Policy, U.S. support serves as a tool of strategic engagement – not global charity. Every U.S. taxpayer dollar is subject to rigorous oversight to ensure measurable results, maximum efficiency, and accountability, with zero tolerance for fraud, waste, or abuse. Through the bilateral Global Health Memorandum of Understanding with Madagascar’s Ministry of Public Health, both governments agreed on shared priorities and responsibilities, including a strong focus on maternal and newborn health and Madagascar’s increasing co-investment in its own health system. Madagascar continues to face major challenges in maternal and newborn health, including high maternal mortality rates and significant gaps in access to quality delivery care. In rural regions, many women still give birth at home or in facilities without adequate infection-prevention supplies. Through the U.S.-funded Momentum Country and Global Leadership Project, the United States is providing essential equipment to healthcare facilities nationwide: calibrated reusable drapes for infection prevention in approximately 1,000 community health centers (CSBs), single-use calibrated drapes for 35 hospitals, and management tools for health workers to improve data collection and care coordination. “U.S. foreign assistance makes America safer by preventing instability and strengthening health systems that can detect and contain infectious diseases before they cross borders,” said Chargé d’Affaires Arnold during the handover ceremony. “Our support is temporary and targeted, designed to help Madagascar build greater self-reliance and reduce long-term dependence on U.S. taxpayer resources.” This U.S. support will help standardize safe delivery practices, strengthen infection prevention, and improve data management systems. Used effectively, these materials will contribute directly to reducing maternal and newborn deaths, especially in rural and hard-to-reach areas where access to quality delivery care remains limited. From major hospitals to remote health centers, U.S.-funded equipment is already delivering results. The nationwide scope of this support ensures that women and newborns across Madagascar benefit from improved infection prevention and safer delivery practices. This targeted assistance helps save lives today while supporting Madagascar’s transition toward a self-sufficient health system, ensuring that today’s U.S. support leads to Madagascar’s future independence from foreign assistance.

United States and Madagascar to Implement $175 Million Five-Year Health MOU - U.S. Embassy in Madagascar

United States and Madagascar to Implement $175 Million Five-Year Health MOU U.S. and Malagasy health officials are convening in Antananarivo for a two-day joint review process, marking the first major step in implementing the strategic health cooperation framework established in the Memorandum of Understanding (MoU) signed between the two countries in December 2025. The MoU outlines the structure and terms of U.S. support to Madagascar’s health sector from 2026 through 2030, with a total commitment of $134,305,000 from the U.S. Government and $41,216,252 from the Government of Madagascar working jointly to address malaria, maternal and child health, polio and global health security issues. This approach reflects the America First Global Health Strategy by prioritizing effective, results-driven cooperation that advances mutual health security and supports Madagascar’s progress toward self-reliance and independence from foreign assistance. The events bring together U.S. Government health experts and Madagascar’s Ministry of Public Health leadership to conduct a comprehensive review of seven major health programs currently supported by United States funding, discussing current activities, and developing recommendations for programs to align with the signed Memorandum of Understanding, which advances both the America First Global Health Strategy and the Government of Madagascar’s goals. “This memorandum sets the course for U.S. strategic cooperation and support to Madagascar’s health sector over the next five years,” said Chargé d’Affaires Stephanie Arnold at the workshop’s opening. This review will help determine how to maximize the effectiveness and impact of these programs, ensuring resources provided by U.S. taxpayer dollars are used wisely,” she added. “By working together to address global health challenges, we demonstrate America’s leadership and commitment to a safer, healthier world, making both the United States and Madagascar healthier and stronger.” Under President Trump’s leadership, the United States is implementing a Global Health Strategy for foreign assistance that emphasizes increased host country ownership, resilient health systems and human resource capacity to reduce dependency, maintaining frontline support to reduce infectious disease threats, and strong performance monitoring systems for accountability and service delivery. “Diseases do not respect borders, and when we support Madagascar’s capacity to detect, prevent, and respond to infectious disease threats, we are contributing to global health security that benefits communities both here and in the United States,” Arnold emphasized, noting that “The MoU represents a commitment to transparency, accountability, and results.” The United States recognizes the commitment of Malagasy leadership in prioritizing investment of time and significant resources on the healthcare of their own citizens. With recent changes in leadership, Madagascar has an opportunity to strengthen governance, improve performance, and move forward toward greater self-reliance, particularly in the healthcare sector. Through strategic focus and thoughtful collaboration under this MoU, U.S. health programs in Madagascar will deliver meaningful results that advance both countries’ interests while supporting Madagascar’s journey toward greater self-reliance and a healthy future.

United States Delivers Life-Saving Health Supplies and equipment to Madagascar

United States Delivers Life-Saving Health Supplies and equipment to Madagascar U.S. Chargé d’Affaires Steve Bremner presided over a handover ceremony to the Government of Madagascar, represented by President of the Refoundation Col. Michaël Randrianirina, marking the delivery of nearly one million mosquito nets, over 900,000 pieces of critical laboratory equipment, and 33 internet connectivity systems. The assistance – valued at more than $2.2 million – is part of a broader U.S.-Madagascar health partnership, outlined in our bilateral Global Health Memorandum of Understanding (MOU) signed in December 2025, totaling $134 million from the U.S. government from 2026 through 2030. “This assistance is clear and disciplined – it is a tool of strategic engagement, not global charity,” said Chargé d’Affaires Steve Bremner. “Every U.S. taxpayer dollar must show measurable results, reduce long-term dependence on U.S. resources, and support greater self-reliance. Stronger health systems in Madagascar help detect and contain infectious diseases before they spread across borders, protecting American and Malagasy families alike.” The assistance includes: 989,250 long-lasting insecticide-treated mosquito nets - worth $2 million – will be distributed to 63 districts and 1,661 community health centers across Madagascar. Malaria remains one of the leading causes of death in the country, claiming the lives of children and women at an unacceptable rate. The third and final delivery of these nets arrived in late June and will be distributed starting this July until mid-October, ensuring that those most at risk of mosquito-borne disease now have access to this basic but life-saving protection. U.S.-funded Global Health Supply Chain Program – Procurement and Supply Management (GHSC-PSM) project will ensure the distribution. 908,523 pieces of critical medical and laboratory equipment – valued at $180,000 – including personal protective equipment, laboratory supplies, diagnostic tools, and cold chain equipment – along with a modernized digital surveillance system to Madagascar’s Ministry of Public Health. This support through the STRIDES (Strengthening Infectious Disease Detection Systems) program bolsters Madagascar’s ability to detect and respond to outbreaks of diseases such as monkeypox, plague, rabies, polio, and Ebola before they become regional or global threats. Over $30,000 of funding for the full procurement, installation, and first year of service for 33 Starlink satellite internet systems at priority districts and regional health offices across the country. These systems will allow health workers in remote and underserved communities to transmit health data quickly to central decision-makers – enabling faster, better-coordinated responses when disease outbreaks occur. U.S.-funded Momentum Country and Global Leadership (MCGL) will implement this project. As the country’s largest bilateral health donor, the United States remains committed to the principle that a stronger, healthier Madagascar makes for a stronger, safer world – and a stronger, safer America.

Chargé Arnold Speech - Implementation Plan of Five-Year Health MOU - U.S. Embassy in Madagascar

Chargée d’Affaires Stephanie Arnold's Speech Discussion on the implementation plan for the five-year health Memorandum of Understanding (…) Colleagues, and honored guests, Good morning, and welcome to the United States Embassy. It is a pleasure to host you today for the second day of this important workshop to discuss the implementation of our bilateral health Memorandum of Understanding on U.S. Government health activities in Madagascar, made possible through the Trump Administration’s America First Global Health Strategy. Today’s focus— discussing a shared vision and developing a preliminary draft for the health MOU Implementation Plan and the terms of reference for the two committees that will steer its implementation—is a decisive step in moving from commitments on paper to concrete action over the next five years. On December 22, 2025, the Governments of Madagascar and the United States signed a five-year bilateral health MOU, with a financial intention of approximately 554 billion Ariary from the United States and This jointly decided MOU intends to strengthen Madagascar’s health system, support lifesaving services, and improve disease prevention and response that matter to both our countries’ health security. For the United States, this means detecting and containing infectious disease outbreaks including malaria, polio, and emerging health threats before they can reach American shores through international travel and trade, protecting American families while reducing the need for costly domestic emergency responses Yesterday, technical experts from the Ministry of Public Health and the U.S. Government worked in thematic groups to draft a preliminary Implementation Plan. Today, your task is to review the shared vision and preliminary draft MOU implementation plan, and the proposed terms of reference for the Joint Steering Committee and the Operational Committee that will guide its implementation. These committees are not just administrative structures. They are the mechanisms that will ensure clear roles and responsibilities, regular dialogue between our two governments, consistent monitoring of progress, and real accountability for the results that will directly benefit the lives of Malagasy citizens. They will also track performance against the benchmarks we have jointly established, which will guide the continuation of U.S. support over the five-year period . Continued U.S. support will be contingent on meeting these performance benchmarks. This ensures accountability to both American and Malagasy taxpayers and guarantees that our partnership delivers measurable results. America First Foreign Assistance means that U.S. support must be strategic, results-driven, and accountable to our taxpayers. It also means we aim to support greater self-reliance and reduced long-term dependence on foreign assistance, with systems that are increasingly financed and led by Madagascar itself. This approach benefits both our nations: Madagascar gains sustainable health capacity, while the United States advances our health security interests and creates opportunities for American health innovations and companies to support Madagascar’s development. In that spirit, I want to recognize the presence of the Ministry’s leadership and the technical directors and program heads responsible for surveillance, basic health services, family health, immunization, countering malaria, laboratories, medicines, and health information systems. Your leadership and technical expertise are essential if this bilateral health MOU is to translate into real and lasting improvements for people in need across the country. Let me emphasize how important collaboration is to the United States in this effort. We can bring American resources, technical expertise, and tools. But this bilateral health MOU can only succeed if it reflects Madagascar’s priorities and if Malagasy institutions lead and maintain its implementation. We see this not as a U.S. project, but as a joint framework under Malagasy leadership, with U.S. support. That is why we are genuinely pleased to host this event here at the Embassy. Opening our doors to you today symbolizes our commitment to work side by side and to remain transparent about how we use U.S. resources in support of Madagascar’s health system. In closing, thank you for your time and your partnership. We are very happy to welcome you here and proud to accompany Madagascar in this next step of implementation planning for our bilateral health MOU. I wish you a productive and focused session. The work you do today will shape how our two governments cooperate on health in the years ahead; it will influence the quality and reliability of health services that Malagasy communities receive; and, most importantly, it will help protect both our nations from infectious disease threats while building Madagascar’s capacity for long-term health system sustainability. Thank you.
